To make a referral to Sycamore Village's memory support program, simply call our 24-hour resource helpline at 618-806-7524. A staff member is available 7 days a week to answer questions, schedule a tour, or set up a move in date.

PRE-SCREENING
A pre-screening by one of our nurses is required prior to moving in to Sycamore Village in order to ensure lifestyle and care needs are met.
Pre-screenings are typically done at Sycamore Village, however, arrangements can be made for the nurse to come to the hospital or home if more convenient.

PHYSICIAN CERTIFICATION
Once family, future resident and the staff agree upon the move to Sycamore Vilalge,a physician's certification form must be completed and signed by the physician overseeing the new resident's care. This is a state of Illinois requirement.
