Help us raise awareness.  Create a video.  Honor a loved one. Tell your story.

Dear Family Member, Friends and Staff:


Sycamore Village invites you to participate in a contest paying tribute to the life of a loved one who lives or lived with a memory loss disorder.  Family and friends are asked to submit a short two minute video tribute of any kind by November 5th in honor of National Alzheimer’s Awareness month.


It may be as simple or as creative as you like. The video may include still photographs, live action, family testimonials, art work, music, or anything else that tells a heartfelt story about that special person.  Your loved one’s life story tribute will be posted on-line and will receive votes from those who view it.


The number of people affected by Alzheimer’s disease or other dementia related disorders is growing and alarmingly, so are those with early on-set.  By linking these videos to Sycamore Vilage’s website and Face Book page, we hope to spread the word to medical professionals, politicians and researchers that more money and manpower are needed to find a cure.


So create a video.  Honor a loved one.  Tell your story.  The winner of this contest will receive a state-of-the-art camcorder and a dinner for four.  Please see below for contest rules and direction.

Instruction and Rules for Video Tribute Contest


The Contest begins on October 1, 2009 and ends on November 5th 2009


Video Requirements:


•The video must not be more than two minutes in length, including titles.
•The video may use any narrative and technical style, including documentary, live action or animation.
•The video must be in English or have English subtitles.
•The video must be uploaded to Sycamore Village's website site at www.sycamorevillage.net/video-tribute-contest by 11:59:59 p.m. CT on November 5, 2009.

OR

•must be submitted on DVD to Sycamore Village staff by 11:59:59 p.m. CT November 5, 2009 to be uploaded by Staff to the website.

 

Each contestant may submit only one (a) Video. If any group of individuals elects to collaborate on a Video, they are required to designate one individual to enter the Contest, be responsible for getting necessary signatures from the group for required documentation, and accept the prize on behalf of the group and appear at the Prize Event on behalf of the group. Sycamore Village is not liable for any disputes between collaborators related to a Video.

 

Once a video is uploaded to the Contest site it is considered final and may not be modified or edited. Video Submissions will not be returned.


You understand and agree that your Video will be posted online and viewed by other people during and after the Contest.

 

IMPORTANT: Be sure to save your Video in the original format after uploading it to the Contest Site. If you are a Winner, you agree that you will provide Sycamore Village with your Video in its original format in order for the Video to be used in future promotion or other activities

 

The Video must meet the following content restrictions:


•    The Video must not contain material that is inappropriate, indecent, obscene, hateful, tortuous, defamatory, slanderous or libelous;
•    The Video must not contain material that promotes bigotry, racism, hatred or harm against any group or individual or promotes discrimination based on race, sex, religion, nationality, disability, sexual orientation or age; and
•    The Video must not contain material that is unlawful, in violation of or contrary to the laws or regulations in any state where Video is created.

 

All Videos will become the exclusive property of Sycamore Village, and none will be acknowledged or returned. By submitting a Video, entrant irrevocably grants The Pointe and its respective agents an unlimited, worldwide, perpetual, unconditional, gratis license and right to edit, modify, adapt, distribute, stream, post, publish, broadcast, reproduce, encode, store, copy, transmit, publicly display, exhibit, and or otherwise use or reuse (without limitation as to when or to the number of times used) the Video and all elements thereof, in whole or in part, including any persons or likeness embodied therein (in each case, as submitted or as edited/modified in any way by the Sponsor, in its sole discretion) in any way, and may use them in any and all media, now or hereafter devised, worldwide in perpetuity, without attribution, compensation, permission or notification to the Entrant, his/her successors or assigns, or any other entity, and without notice, review, or approval from Entrant or any other party.

Judging

Using an on-line ranking tool, viewers will have the opportunity to rank (1 through 3 with 1 being the highest ranking)  Instructions for using the Voting Tool will be provided. The Entry with the highest vote total will win the Grand Prize (defined below). In the event of a tie, all tied entries will be re-judged by an additional judge selected by Sycamore Village who, using the judging criteria based on (1) degree to which it inspires others; (2) creativity and style; and (3) production quality, will break the tie.

Decisions are final and binding in all matters related to this Contest. Winning a prize is contingent upon fulfilling all requirements.

Publicity

By participating, where allowed by law, all participants and winner(s) grant the Sycamore Village exclusive permission to use their names, characters, photographs, voices, and likenesses in connection with promotion of this and other contests and waive any claims to royalty, right, or remuneration for such use.

Prize

Winning entry will be awarded 1) state of the art HD Camcorder and 2) dinner for 4 ($150.00 value).

Winner will be notified by November 9, 2009.
